Step 1: Complete the Online Application

- Create an account to submit your student’s application

🔵 Current 2025–26 School Year (Aug 2025 – June 2026)
🟢 Upcoming 2026–27 School Year (Aug 2026 – June 2027)
Step 2: Submit required documents in Let’s talk
⏰ Deadline: July 24, 2026
- Students cannot start school until all documents are received and approved.
- All the required documents must be submitted by the deadline or your student may not start the first day of school.

☐ Verification of Residency and Statement of Residence (English) (Español)
☐ Two utility bills (Click here)
☐ Immunization Record (English) (Español)
☐ Proof of Birth (TK–1 required)
- Birth Certificate, Passport, Baptismal Certificate, Proof of Birth Affidavit
- Hospital Certificate is NOT acceptable
☐ If you are not the parent, please provide Guardianship or Foster paperwork

- Review and check appropriate risk factor boxes. Please fill out the highlighted section on both pages with the student's information. The rest of the form may be completed by the student's primary care provider or may be completed by a school nurse.
