California Healthy Kids Survey
Each year students in 5th through 8th grade have the opportunity to participate in the California Healthy Kids Survey (CHKS). The survey includes questions about alcohol, tobacco, and other drug issues, as well as questions about school safety, mental health supports, and the learning environment.
Student participation in the survey is voluntary. The survey is anonymous. No names or any other identifying information is connected to the answers except for the name of your child’s school. For students in 5th and 6th grade, permission to participate in the survey must be provided by a parent or
guardian. For students in 7th and 8th grade, parents or guardians have the option to opt out of participation in the survey.
Parents of students taking the survey will receive an electronic notification/permission slip. If you have questions, please click Let's Talk and connect with a member of the Educational Services Department.
